7 page survey stuck on page 4

I have a 7 page survey using v3.0.4. When I get to page 4 and click NEXT, the next page is a repeat of page 4. The page 5 questions never come up. I can keep hitting next and the page stays on #4. The page number on the progress bar in the upper right keeps increasing even though the page is staying the same. Any idea what might be wrong?
