Cannot access reports

I have started building my first survey using the extension.

I had some initial problems viewing the survey which I solved by reloading the GeoLiteCity.dat as you suggested in response to another forum question.

I can now access the survey on the front end, answer the questions and finish the survey. I then get the consolidated results when I finish the survey.

BUT when I try and access the "Report" via the front-end or back-end I get a blank page. Also when I click on the "responses" in the back-end I also get a blank page.

When I turn on error reporting I'm getting these error messages:

Is there something I've installed wrongly or is there a fix for this?
