Matrix

Hello. Thank you for such a wonderful piece of software. I would like to purchase this software but I need to confirm that I can have (in a matrix format) a list of "features" in one column, a yes/no toggle asking if they use this feature in the second column, a rating (1-5) of that feature in that third column and a multiple choice in the forth column. (See attached image for clarification.)

Column one: Vertical list of features
Column two: Do you use this feature? (boolean)
Column three: How much do you like this feature? (rating 1-5)
Column four: In which application do you use this feature? (multiple choice)

Is it possible to allow a user to add their own question or "feature"?

Please let me know if this is possible. If not, are you willing to do custom work?
