Menu link to a Single Survey not working

Hi, I created a menu link to a single survey but when I click on the menu item, the survery does not show. I also tried to make a menu to a Survey List. The list appeared but when I try to click the title of the survery, the form does not load. Would appreciate some assistance.  You can check it at https://www.pcs-cacontrol.org. Thanks.
