Wrong layout + wrong default date in J 1.6.1

Hello,

Following problems in a Joomla 1.6.1 installation:

1: layout error
In the creation screen for a survey, the icons of the survey components are not correctly displayed which makes it difficult to read the text (see print screen in attachment

2: If the fields start- and endate are left empty, the date 1901-12-13 20:00 is entered.

3: If the field "maximum allowed responses" is left blank, the value '0' is entered, making the creation of responses impossible
