Error: Blank Screen on DashBoard and Surveys

We've recently been experiencing some issues with Community Surveys.

We receive a blank screen on the bank end dashboard and also receive the same blank screen in the front end when we try to access a survey that’s been created.

Has anyone got a fix for this issue?
