I have upgraded to Joomla 4 including the latest version of Community Surveys. Either with my new template or the default Joomla Cassiopeia the layout looks messed up. It doesn't matter what 'default layout' I choose it looks the same. I haven't made any changes since Joomla 3 but of course, I'm using a different template. Essentially, the four choices for each answer are stacked, rather than tabbed out in a row. Is there a simple way to make my form look like your demo? See images attached.
