Community Survey 6.5.4: Database error and JS error

Hello,
I am experiencing some errors on live site and localhost after updating to 6.5.4: database error in the backend that cannot be fixed by clicking repair. And JS error in the frontend: Uncaught ReferenceError: L is not defined.
