Hello,

Refer attached screenshot which i am getting when i click on start survey.

Steps to reproduce - 

1. Go to - https://www.anildagia.com/personal-outcomes-form-icf
2. Click on Start Survey
3. Immediately I see this error

Then if I do a browser refresh and then again click on Start Survey - then this type there is no error.

But my clients who are required to fill up this questionnaire have reported that they get this error at the time of marking the survey as over.
