I just purchased and installed the component and am having some issues with creating a survey. The site I am installing on is 100% public access, no login from the front end. I need to be able to create surveys for public access on hidden pages. Here is the page I am trying to develop:

I am not sure about what permission levels to set in the component, but I need help!!!
