A few "how to" questions I couldn't find in FAQ

Hello all.
- how to allow users to add comments to their survey answers?
- how to restart a survey (users are allowed to take survey again even with Restriction=Username)?
- is there a way to have an empty line between questions (page header would do it without the symbol)?
- can I have two single line questions next to each other (same column)?
