I can no longer create new surveys or copy old ones.  

Versions in use:

Community Surveys 5.1.2 and CjLib 2.8.8

Running Joomla 3.9.6

My account is in good standing.

When I click new, it opens the correct screen, but when saved, doesn't save the survey.  When I attempt to copy an old survey, it closes the window, shows the survey I attempted to copy as checked out, and no copy of the survey is found in the list.

I would appreciate any help you could provide.  We've been using this extension for 3-4 years and really like it.
