installation and continues error log

hi i am have a working copy of this component on our site, but i noticed that the component community survey on the joomla component menu does not have an icon similar to the other components.

also on our apache error logs there is a persistent error regarding community survey.
[Wed Jan 02 17:54:18 2013] [error] [client 49.144.15.183] File does not exist: /opt/bitnami/apps/joomla/htdocs/components/com_communitysurveys/assets,

i tried to install this component on a fresh joomla 2.58 install still i get the same no icon on the menu
