Is it possible to add survey forms to Easyblog articles?

Hi,

I want to insert a survey into EasyBlog 5 articles but not sure how I do this. I have tried by creating my own module position and using Joomla load module position in the blog article but this doesn't work.

Do you have a way of achieving this?
