NOTES:

This will be used for peer evaluations in a class in which students watch, perform, and then evaluate each other. As with survey software, I’d like the results available live, directly after submission.

Each class is comprised of eight teams, so whatever appears in the examples would need to be duplicated eight times, i.e., Team 1, Team 2, etc.
