Menu Bugs

We purchased the Community Polls and are having some issues implementing them on the website, as listed below:

1. When I try to create a poll, and hit the submit button, the cursor goes to the the top of the page and does not submit the poll. (problem has been repeated in chrome and firefox)

2. If I press enter after filling out the fields, the poll submits, but if try to view it I get an fatal error page. The control panel also shows an error. I have attached screenshots of both.

Please help us get your poll module working on our site. Thanks.
