Number formats

Hello Guys,

I recently purchased the component and I am on my first survey configuration.

I need you point me on the right track with this. I want to create a survey where I will let the person type the name of a responsible, address, email and phone number.

I found a way to select the forms for name, email and address but I have not been able to find a way to expose the phone number.

On a different page, I will also need the person taking the survey type a figure in number format.

Can you help me out with this?
