Question before i Purchase

I am interest in community surveys but I first want to know , will i need coding experience to be able to create a survey such as this one you have as demonstration

Where you have the address fields next to each other with the titles for each field named inside the field? or is there options on the component to do this? If not , how would i go about doing the same as the demo link?
