Radio Grid does not display all columns

Hi, I am using the latest versions of Community Surveys and CjLib2.9.6 on a Joomla3.9.13 site.

I have setup a survey using a Radio Grid question. The question displays properly in the preview with 2 columns but not on the live survey

See the last question 'Boat Ownership' at https://sbsc.net.au/index.php/the-club/introduction/test-survey-only

What might be causing the live survey to not display the radio grid question in 2 columns?

PS I have attached a screenshot in case the survey is not public
