I have just updated our site to Joomla 3.6.0 and at the same time updated Community Surveys to 4.2.1 (and also the CJlib to 2.4.8), We were still on Community Surveys 3.8.1 (I think - it was definitely 3. something).

At first I couldn't see any of the surveys, just the categories, so I went to migrate and migrated it. Then the surveys appeared. I can see the surveys, the results and everything fine on the back end.

BUT on the front end where people used to access the survey it no longer works. I had a menu with "survey response form layout" on it. I've tried updating the survey key but that makes no difference. Basically when you go to the survey you get this:
